I am a sole trader operating as Nadine Hutchinson Dyslexia Assessment & Support, providing dyslexia assessment, dyslexia screening consultation and educational consultancy services within the United Kingdom.

I am registered with the Information Commissioner’s Office (ICO) as a Data Controller.
2. The information I collect
For the purpose of providing a dyslexia assessment, dyslexia screening consultation or educational consultancy service, I may collect and process the following information:
- personal details, including name, date of birth, address, parent/carer contact details and school
- background information, including developmental and educational history
- relevant medical, health or developmental information that you choose to provide
- information from parents/carers, schools or other professionals, where provided
- results from standardised tests, screening tasks or other assessment activities
- professional notes and observations
- copies of relevant reports or documents you choose to share with me
- written reports, summary profiles or recommendations produced as part of the service
Some of this information may constitute special category data under UK GDPR, for example health or developmental information.
3. Lawful basis for processing
The lawful basis for processing personal data is:
Article 6(1)(b): Contract — processing is necessary to provide the agreed assessment, screening consultation or educational consultancy service.
Article 6(1)(c): Legal obligation — where processing is necessary to meet legal, tax, safeguarding or regulatory responsibilities.
Where special category data is processed, such as health or developmental information, the condition for processing is:
Article 9(2)(a): Explicit consent — you give explicit consent for me to process relevant special category data for the purpose of providing the agreed service.
You may withdraw consent for the processing of special category data. However, if consent is withdrawn, I may not be able to continue with the assessment, screening consultation or consultancy service. Some records may also need to be retained where there is a legal, safeguarding, insurance or professional reason to do so.
4. How your information is used
Information gathered will be used to:
- arrange and provide the agreed service
- complete a dyslexia assessment, dyslexia screening consultation or educational consultancy review
- understand your child’s learning profile, strengths and areas of difficulty
- prepare a written diagnostic report, screening summary profile or written guidance, depending on the service agreed
- communicate findings and recommendations to parents/carers
- provide practical strategies and next steps
- meet legal, regulatory, tax, insurance and safeguarding obligations
- keep a professional record of the work completed
For a full diagnostic dyslexia assessment, a detailed written report will be produced.
For a dyslexia screening consultation, a shorter summary profile will be produced. A screening consultation is not a full diagnostic assessment and cannot provide a formal diagnosis of dyslexia.
It is the parent/carer’s responsibility to decide whether to share the report or summary profile with school or other professionals, unless you have given me explicit consent to share it directly.
Your data will never be sold. I will not use your personal information for marketing purposes unless you have given consent for this.
5. Storage and retention of data
- Digital files are stored securely on password-protected devices and/or secure cloud storage.
- Paper notes and record forms are stored securely while required.
- Working notes and paper records are securely shredded when they are no longer required.
- An electronic copy of the final diagnostic report or screening summary profile is retained securely for six years from the date of the report or summary.
- Relevant administrative records may be retained for legal, insurance and tax purposes.
- After the retention period, records will be securely deleted.
6. Sharing your information
Your child’s information will only be shared:
- with your explicit consent, for example if you ask me to share a report or summary profile with school
- when required by law, for example by court order
- where there is a safeguarding concern and I have a legal or professional duty to share information
- with relevant professional bodies, insurers or legal advisers where necessary for verification, quality assurance, complaint handling or professional protection
Where possible, I will discuss this with you first. However, in safeguarding situations, information may need to be shared without consent if this is necessary to protect a child or young person.
7. Safeguarding
If, during the assessment, screening consultation or consultancy process, I become concerned about the safety or wellbeing of your child, I have a professional duty to follow safeguarding procedures.
This may require sharing relevant information with appropriate authorities in line with safeguarding legislation and guidance.
8. Your data protection rights
Under UK GDPR, you have rights in relation to your personal data and your child’s personal data. These may include the right to:
- access the information held
- ask for inaccurate information to be corrected
- ask for information to be deleted, where there is no legal or professional reason to retain it
- ask for processing to be restricted in certain circumstances
- object to processing in certain circumstances
- withdraw consent where consent is the lawful basis or condition for processing
- complain if you are unhappy with how your information has been handled
Some records may need to be retained to meet legal, safeguarding, insurance or professional obligations.
